Work like a medical laboratory scientist: accept or reject specimens, read chemistry, hematology, microbiology and blood bank results, call critical values, run daily QC and plan your certification.

Clinical Laboratory Practice

Write a fictional patient (name, MRN, unit) and a result that is critical on the illustrative list, for example glucose 38 mg/dL.

Write three verification checks you would do before calling (specimen quality, QC status, patient history), and what you would find in this scenario.

Write a script for the call: who you ask for, what you say, and the exact words for the result with units.

With a partner (or reading both parts aloud), act the call, including the read-back. Have the partner deliberately mishear one detail and correct it.

Write the LIS documentation line: value, person and role called, time, read-back confirmed, your initials.

Draw an escalation path with three levels for when the caregiver does not answer, and note how many minutes you would wait at each level (use a made-up policy).

Write a second scenario in which the 'critical' result is an artefact (for example a hemolyzed potassium). Write what you would do instead of calling it.

## Why it matters

Some results mean a patient is in danger right now: a potassium high enough to stop the heart, a glucose low enough to cause a seizure, a positive blood culture in a newborn. These are **critical values** (sometimes called panic values). The lab's job does not end when the number is on screen. It ends when a responsible caregiver has **received, understood and acknowledged** the result, and that is documented. Delayed or failed critical-value calls are a recognized cause of patient harm, and accreditors inspect this process closely.

> **Scope.** The critical limits below are **illustrative teaching values only**. Every lab's medical director and medical staff approve the lab's own list, which you follow. The activity is a paper role-play with fictional patients. Nothing here is medical advice.

## The core idea

A critical-value process has five parts:

1. **A written list.** Tests and limits, approved by the lab director with the medical staff, sometimes with different limits for newborns or for repeat results.
2. **Verification, without delay.** Check that the specimen is acceptable (no hemolysis, clot or contamination pattern), the instrument QC is in control, and the result fits the patient history. Some labs repeat the test; others do not, because a repeat delays the call. Follow policy.
3. **Notification within a set time.** Call the responsible licensed caregiver (the ordering provider or the patient's nurse, as the policy defines) within the lab's time limit, often expressed in minutes.
4. **Read-back.** The receiver repeats the patient's name, second identifier, test and value back to the caller. This catches mishearing ("fifteen" vs "fifty").
5. **Documentation.** In the laboratory information system (LIS): who was called, their name and role, date and time, the value, and that read-back was completed.

If the caregiver cannot be reached within the time limit, the lab follows an **escalation path**: for example the charge nurse, then the covering provider, then the lab medical director.

## Worked example: a call, start to finish

At 02:14 the analyser reports potassium **6.9 mmol/L** for a fictional inpatient, Sam Ortiz (MRN 40417). The MLS:

1. Checks the specimen: no hemolysis flag, plasma looks normal, no EDTA pattern (calcium normal). QC from 01:30 is in control. The previous potassium was 6.0 yesterday, and creatinine is high, so the result is plausible.
2. Calls the patient's unit at 02:17 and asks for the nurse caring for Sam Ortiz.
3. Says: "This is the lab with a critical value for Sam Ortiz, MRN 40417. Potassium is 6.9 millimoles per litre, collected at 01:58."
4. The nurse reads back: "Sam Ortiz, 40417, potassium 6.9." The MLS confirms.
5. Documents in the LIS:

Total time from result to documented call: five minutes. If no one had answered by the time limit, the MLS would move to the next person on the escalation path and document each attempt.

## Reporting beyond critical values

The post-analytical phase also includes:

- **Autoverification:** LIS rules release normal results that pass every check (QC in control, no flags, delta check passed) without manual review, so staff can focus on the exceptions.
- **Corrected reports:** if a released result was wrong, the lab issues a corrected report, notifies the caregiver, and keeps the original visible in the audit trail. Results are never silently overwritten.
- **Comments:** standard comments ("hemolyzed specimen; potassium may be falsely raised") travel with the result.

## Common mistakes

- Leaving a voicemail or message with a clerk instead of speaking to a licensed caregiver.
- Skipping read-back, or documenting "called" without a name and time.
- Delaying the call to repeat a test when policy does not require it.
- Calling a critical value that is clearly an artefact (an EDTA-contaminated potassium) instead of requesting a redraw.

## Why it matters

Before any patient result leaves the lab, the lab must show that the method was working when that result was produced. **Quality control (QC)** does this by running control materials with known target values alongside patient samples. In the US, the CLIA regulations require QC for nonwaived tests, typically at least **two levels of control each day of testing** for most quantitative chemistry tests, unless the lab uses an approved alternative such as an Individualized Quality Control Plan (IQCP) ([CMS CLIA](https://www.cms.gov/medicare/quality/clinical-laboratory-improvement-amendments), checked on 2026-10-23). Statistical QC basics were covered in `lab-data-quality`; this lesson applies them to the daily clinical decision: **accept the run or reject it**.

> **Scope.** All data here are fictional. QC limits, rules and frequency are set by each lab's director and procedures.

## The setup

- Each control level has a **target mean** and **SD**, established by the lab from repeated measurements of that lot of control material.
- Each result is converted to a **z-score**: z = (result - mean) / SD.
- Results are plotted on a **Levey-Jennings chart** with lines at the mean and at +/-1, 2 and 3 SD.
- Two levels are chosen near clinical decision points, for example a normal potassium and a high one.

## Westgard multirules, across levels

Using two levels lets the lab apply rules **across levels within a run** and **across runs within a level**:

**Random error** (1_3s, R_4s) looks like scatter: a bubble in a pipette, a clot, an air leak. **Systematic error** (2_2s, 4_1s, 10x) is a shift or trend: a new reagent lot, calibration drift, a deteriorating lamp.

## Worked example: this morning's potassium run

Level 1 (mean 4.00, SD 0.10) reads **4.23**: z = +2.3. Level 2 (mean 6.50, SD 0.15) reads **6.17**: z = -2.2.

- 1_2s: yes (both beyond 2 SD), so check the rest.
- 1_3s: no. 2_2s: no (they are on opposite sides).
- **R_4s: yes.** The spread between the two levels is 2.3 + 2.2 = 4.5 SD in a single run.

Decision: **reject**. Random error is likely. Patient results from this run are not released.

## What to do when a run is rejected

1. **Do not report** patient results from the run.
2. **Do not** simply rerun the control until it falls inside the limits. That hides the problem.
3. **Troubleshoot** according to the error type: for random error, look at sampling, bubbles, clots and the specific control vial; for systematic error, look at reagent lots, calibration and maintenance.
4. Fix the cause, recalibrate if needed, and **rerun both control levels**. Accept only when the run passes.
5. **Re-test patient samples** run since the last acceptable QC if they may be affected, and issue corrected reports for any released results that change.
6. **Document** the rule broken, the cause, the corrective action and the patients affected.

## Common mistakes

- Rejecting runs on a 1_2s warning alone, which with two levels rejects too many good runs, or ignoring 1_2s entirely.
- Using the manufacturer's wide ranges instead of the lab's own mean and SD.
- Repeating controls until they pass.
- Forgetting to look back at patient results since the last good QC.

Write down your current education (degree, major, and science and math courses completed).

Use the program search on the NAACLS website (https://www.naacls.org) to find two or three accredited MLS or MLT programs you could realistically attend. Note their format (four-year, post-baccalaureate, online with local clinical sites) and length.

For one program, list its prerequisites and mark which ones you have and which you still need.

On the ASCP Board of Certification site (https://www.ascp.org/boc), find the eligibility route that matches your plan and note what documentation it requires.

Find out whether the state where you want to work licenses laboratory personnel. If it does, find the official licensing page and list its requirements.

Note the date you checked each source, because requirements change.

## Why it matters

The platform can teach you how a clinical laboratory works. It cannot make you a medical laboratory scientist. In the US, MLS work requires an accredited education with **clinical rotations**, a national **certification exam**, and in some states a **state licence**. Planning that route early saves years: picking the wrong degree or an unaccredited program can mean repeating coursework.

## The roles

- **Medical laboratory scientist (MLS)**, also called medical technologist (MT) or clinical laboratory scientist (CLS): usually a **bachelor's degree** and a NAACLS-accredited MLS program. Performs and supervises high-complexity testing across departments.
- **Medical laboratory technician (MLT):** usually an **associate degree** from an accredited MLT program. Performs many tests, often with more limited responsibility for complex testing. Many MLTs later complete an MLT-to-MLS bridge program.
- Specialist certifications exist in areas such as blood banking, microbiology, chemistry and molecular biology, usually after experience.

## Education: NAACLS accreditation

The **National Accrediting Agency for Clinical Laboratory Sciences** ([NAACLS](https://www.naacls.org)) accredits MLS and MLT programs. Accredited programs combine coursework in chemistry, hematology, microbiology, immunohematology, immunology and molecular methods with a supervised **clinical practicum** in a working hospital lab. Some programs are a full four-year degree; others are a one-year post-baccalaureate certificate for people who already hold a science bachelor's degree with the right prerequisites. NAACLS keeps a program search on its site.

## Certification: ASCP Board of Certification

The **ASCP Board of Certification** ([ASCP BOC](https://www.ascp.org/boc)) offers the MLS(ASCP) and MLT(ASCP) credentials. Candidates qualify through an **eligibility route**: the most common is completing a NAACLS-accredited program, while other routes combine a degree with documented clinical experience. The exam is computer-adaptive and covers the laboratory departments in this course. Certification must be maintained through ASCP's **Credential Maintenance Program** with continuing education. Check the BOC site for the current routes, content outline and maintenance cycle rather than relying on a summary.

Other certifying bodies exist (for example AMT), but many employers and most state licensing boards recognize ASCP.

## State licensure

Most states do not license laboratory personnel separately, but a number do, including **California, New York and Florida**. In those states you need a state licence (which usually requires certification or a state exam plus specific education) before you can work. Rules differ by state and by job level, so look up the state's health department or licensing board directly.

Federal **CLIA** regulations also set minimum personnel qualifications for testing at each complexity level ([CMS CLIA](https://www.cms.gov/medicare/quality/clinical-laboratory-improvement-amendments)).

## Worked example: planning a route

A fictional learner, Dana, has a biology bachelor's degree and lives in Florida.

1. Dana compares her transcript with the prerequisites of nearby **post-baccalaureate MLS programs** listed on NAACLS (typically chemistry including organic or biochemistry, biology including microbiology, and math or statistics). She is missing one course and plans to take it next term.
2. She applies to a 12-month accredited program that includes clinical rotations.
3. After completing it, she qualifies for the **MLS(ASCP)** exam through the accredited-program route.
4. Because Florida licenses lab personnel, she checks the state board's requirements and applies for her licence once certified.
5. She sets a reminder for credential maintenance and continuing education.
